Indiana is a direct access state, meaning many patients can begin physical therapy without a physician referral.
If a referral is needed based on your individual situation, I'll help guide you through the process.

Rooted is a private-pay physical therapy practice and does not bill insurance.
Cash, check, credit/debit cards, HSA/FSA cards, Apple Pay, Venmo, and PayPal are accepted. Secure electronic payments are processed through Square. Payment is due at the time of service, and itemized receipts are provided for all visits.
This allows me to provide:
Longer one-on-one visits
Care in the comfort of your home
Personalized treatment plans
No visit limits or insurance restrictions
Flexible scheduling
Many patients choose Rooted because they value individualized care and direct access to their physical therapist.
Upon request, I can provide a detailed superbill that you may submit to your insurance company for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual insurance plan.

Standard pricing applies within Hamilton County. A travel fee of $20 per visit may apply for appointments outside of Hamilton County. Any applicable travel fee will be discussed prior to scheduling services.
